HEART CLINIC SERVICES
Our comprehensive cardiac treatment options cover a wide spectrum of conditions and diseases affecting the heart and vascular system. As healthcare advances, new diagnostic techniques, drugs and interventions are continually being integrated into The Heart Clinic’s cardiovascular care.
NEW PATIENTS
The Heart Clinic welcomes new patients and physician referrals. We work closely with your other healthcare professionals to coordinate your cardiology care with your other medical issues. In most cases, we are able to schedule new patient evaluations or consultations within 24 hours. A new patient referral from friends and family members is the highest compliment we can receive. Your recommendations are greatly appreciated and encouraged.
YOUR FIRST APPOINTMENT
On your first visit, we appreciate if new patients arrive 15 minutes early in order to complete patient registration and insurance forms. To help expedite the registration process, please bring the following with you to your first appointment:
- Current health insurance card
- A photo I.D.
- A list of current medications and dosages
- Any previous cardiology medical records and electrocardiograms (EKGs)
- Any recent labs or other medical tests
If you would like to print out patient registration and insurance forms to complete before your first appointment, please download a new patient form.
HMOS AND REFERRALS
Your insurance plan may require a referral prior to being seen by one of our physicians. We will be happy to assist you in this process. If your referral is not complete by your appointment time, we are happy to reschedule for a later date if necessary.
BILLING AND INSURANCE
At The Heart Clinic, we accept most major health insurance plans. Please note that if your insurance plan requires a co-payment and/or deductible, it will be collected at the time of check-in. If you have questions regarding insurance or other billing issues, please contact our Billing Department at 770-732-2870.
Scheduling
To schedule an appointment at either office, please call 770-732-2882. If you must cancel or reschedule your appointment, we appreciate if you would give us at least 24 hours notice.
Prescription Refills and Nursing Calls
Our registered nurses and medical technicians are experienced in the field of cardiology and work closely with our physicians to address your cardiology questions and concerns. In addition, we are happy to assist with prescription refills for cardiology medications prescribed by our physicians. If you have a medical problem which needs immediate attention, our operator can connect you with our clinical staff. If you are experiencing an emergency, please call 911 immediately.
